The Home Care Team:
RN Case Manager
This licensed and trained professional coordinates and manages the patient’s care, evaluates their condition and needs, and implements the Physician’s treatment plan.
Skilled Nurse
Assists the RN Case Manager in carrying out the treatment plan, providing care, educating and monitoring the condition of the patient.
Rehabilitation Services
This includes Physical, Occupational and Speech therapists who work closely with the care givers and patient to achieve a higher level of independence.
Medical Social Worker
Assists patients and families with accessing financial resources, emotional and community support and counseling, and transitioning to a new environment such as assisted living or a relative’s home.
Certified Home Health Aides
Assists the patient with bathing, grooming, ambulation and activities of daily living.

Who Pays for Home Health Care?
Home Health Care is a benefit under Medicare Part A with no co-pay or deductible. Most insurance companies pay for home health care at 80 – 100% depending on circumstances. Medicaid and other programs pay for home health care under certain circumstances. Lifecare Oklahoma will verify and authorize insurance and other benefits on behalf of our patients. |
